Role Description
This is a contract role for a Support Coordinator/Specialist Support Coordinator/Social Worker. As part of our team, the specialist will be responsible for providing support coordination, case management, and recovery coaching to our clients. The role involves developing personalized plans, facilitating access to services, and offering ongoing support to ensure clients' needs are met. This is a remote role, allowing for flexible working arrangements while maintaining constant communication with clients and service providers.
Qualifications
- Strong Interpersonal Skills and Communication abilities, both verbal and written
- Experience in Case Management as well as Support Coordination
- Technical Support skills and proficiency with relevant tools
- Ability to work independently and remotely
- Excellent organizational and problem-solving skills
- Previous experience in the NDIS sector is essential
- Bachelor's degree in Social Work, Psychology, or related field is preferred
