The organization is a specialty care management provider focused on improving outcomes for people with complex injuries and diagnoses. Operating since 1991, it specializes in value-based care through clinical solutions such as risk-based case management, specialty networks, home health, shared decision support, and payment integrity programs.

About the Opportunity

The Referral Care Coordinator role supports injured workers by coordinating specialty network referrals and acting as a central liaison among injured workers, clients, and providers. This position ensures timely, high-quality care by managing appointment follow-ups, obtaining medical documentation, and providing clear status updates to meet service expectations. The role is fully remote, operating Monday through Friday, 9:00 a.m. to 6:00 p.m. Eastern Time.

Responsibilities

  • Provide customer service via phone and email with clients, providers, and injured workers.
  • Serve as liaison between clients, providers, and injured workers.
  • Follow standardized care coordination processes to meet client service agreements.
  • Obtain medical information such as attendance records, appointment details, and medical notes.
  • Reschedule missed appointments.
  • Deliver timely information, reports, and status updates to clients.
  • Collaborate with operations staff to manage referrals and escalate delays as needed.

Requirements

  • Strong customer service skills.
  • Experience in a medical office setting is advantageous.
  • Excellent oral and written communication skills and phone presence.
  • Ability to work independently.
  • High attention to detail.
  • Proficiency with Microsoft Office and computer applications.
